The Director of Healthcare Social Services responsibility is to oversee work of Healthcare Social Services Program. Provide leadership and coordination of social services team. Responsible for promoting self-determination and quality of life in older adults and providing support for all Healthcare residents.
Position Responsibilities:
- Provide direct supervision for department team members, including task assignment, goal setting, coaching/counseling and required performance appraisals. Assure evaluations are completed within required time frame. Foster teamwork among the team members.
- Responsible to perform direct Social Worker functions to assigned units.
- Maintain oversight for Behavior Team and Care Planning team meetings assuring appropriate
attendance recommendations and follow through. Monitor meetings for effectiveness and
appropriateness. Monitors follow up on concerns and recommendations with nursing, MDS
coordinators and other team members as appropriate. - Work cooperatively with the Nursing department to enhance resident experience.
- Work cooperatively with the IL and Assisted Living Resident Service Coordinators (RSCs) to
ensure a smooth transition in the delivery of social services for campus residents as they move into and out of Healthcare. - Serve as backup for Admissions.
- Participate in the on-call, week-end and holiday rotation.
Position Requirements:
- Bachelor’s degree in Social Work, Sociology, Psychology or related field; Master’s degree preferred.
- Five or more years of long-term care experience required. Familiarity and working knowledge of
issues facing older adults in health care. - Previous supervisory experience of 3 - 5 years.
- Familiar with Medicare, Medicaid, VA and pre-admission screening programs.
- Licensed/Obtain licensure as a Social Worker.
Schedule:
- Day shift, Monday-Friday
- Some weekend hours
- Weekend management rotation
