Schedule Monday - Friday
Pay $47,840 - 56,160 Annually -PLUS INCENTIVE
(This is an hourly position/ Based on a 40 hour week)

SUMMARY: The Service Coordinator enables an eligible child and the child’s family/caregivers to receive the rights, procedural safeguards, and services authorized to be provided under the State’s Early Intervention system.
ESSENTIAL FUNCTIONS:
- Determine services in collaboration with families using Individualized Family Service Plans.
- Perform home visits, screening, assessment and educational group activities in the natural environments while adhering to all Massachusetts Department of Public Health Early Intervention Operational Standards.
- Attend parent meetings, community case conferences, transitional activities or relevant activities that may include providing insight, information, referral or advocacy for child and family.
- Maintain and update all clinical records in the Early Intervention Client System.
- Provide co-treatment capacity consultation to other team members in matters requiring discipline expertise.
- Prepare for Certification as an Early Intervention Specialist within three years of employment.
- Maintain expertise through attending meetings, in-services, continuing education or other programs.
- Maintain assigned productivity level on a monthly basis.
QUALIFICATION:
- Bachelor’s or Master’s degree from an accredited institution as determined by the Operational Standards.
- Knowledge and skill obtained through at least two years of experience in human services.
- Skill in communicating and working both independently and within a team.
- Ability to provide current copy of all required professional licenses, CPR, first aide and health records.
- A qualifying criminal background check, driving record and a valid driver’s license required.
