SUMMARY: As key member of multi-disciplined FamilyWorks team, the FamilyWorks Coach is responsible for engaging and increasing CHA residents’ self-efficacy and ability to effect positive behavior change. Employing strength-based & trauma-informed tools and resources, Coaches motivate and problem-solve with individuals and their families to identify, understand and develop actionable strategies that address and resolve barriers and stressors.
COLLABORATES WITH: Collaborations and work relationships exist with: YWCA leadership and staff; CHA FamilyWorks staff; Volunteers/Interns; CHA Leasing Office staff and other partners.
ESSENTIAL RESPONSIBILITIES:
Program Operations (75%): Establish and maintain friendly, compassionate, professional working relationships with individuals receiving services, family members and support team in a manner that reflects the agency’s values, core principles and culture. Collaborate with FamilyWorks Team to create an engaging environment that keeps families engaged and wanting to be involved, ensuring all interactions lead to valued experiences and family outcomes.
Utilize information gathered from assessments, motivational interviews, service partners and other sources to determine the families’ strengths, aspiration, priorities, challenges and needs to develop skills in independent problem-solving and functioning through measurable goals and support; Facilitate individual, family and group coaching by encouraging sharing, discussion and learning from one another with particular emphasis on safety and building self-efficacy.
Facilitate the completion, semi-annual review, and periodic reviews as needed, of family driven Individual Family Service Plans and Personal Support Plans in compliance with FamilyWorks Model, adjusting strategies and expectations as needed; Provide coaching updates to FamilyWorks Site Manager and multidisciplinary team members in a timely manner to monitor program performance.
Assist with orientations and enrollment of eligible resident; Develop and implement engaging workshops and trainings that optimize skill and knowledge attainment of clients. Provide crisis intervention and support to individuals and families experiencing crisis that impairs their capacity to function and make informed decisions.
Assure the implementation and monitoring of the service plan by documenting visits and contacts, maintaining accurate and thorough documentation of contacts according with program procedures and guidelines.
Outreach and Collaborations (10%): Develop positive relationships with key stakeholders, including CHA leadership and staff, CHA residents, Local and Central Advisory Councils, CHA leasing office staff and other community partners, allies and champions to cultivate and activate strong partnerships that support strong programming/services and innovation. Attends internal and external meetings for the purpose of developing a YWCA presence in the community. Perform designated public speaking engagements and media relations. Serves as a knowledgeable resource for information about FamilyWorks Program and achievements and barriers experienced by CHA residents.
Data Management & Reporting (5%): Maintain a high level of organization for file maintenance, daily, weekly and monthly recordkeeping, completing all work within established timelines; Responsible for entering program information into the program database according to schedule. Assist with FamilyWorks intake process. Assist in the maintenance of confidential client files, conduct file reviews, and case reviews. Provide program reports based on funding requirements and assist in development of related documentation to use for program evaluations. (15%)
Professional Development (5%): Complete six hours per year of continuing education in providing family-centered and trauma-informed services. Complete other continuing education and training as set by YWCA or other agencies/funders.
Growth and Innovation (5%): The FamilyWorks Coach collaborates with diverse internal and external stakeholders to design, pilot and evaluate innovate programs and activities. Actively contribute to the continuous improvement of FamilyWorks services, systems and resources with a particular focus on family centered practice; Understand and interpret YWCA business strategy, service model and organizational priorities and design to implement innovative solutions that align with clients and other end users expectations and experiences.
NON-ESSENTIAL RESPONSIBILITIES: Assist with general office functions, including distribution of materials/reports; mail preparation and distribution; assembling/distributing supply packets. Participate in meeting coordination. Support community fairs, events and gatherings. May work with volunteers.
OTHER DUTIES AS ASSIGNED: Other duties outside of the responsibilities of the position may be assigned, whether for a project, special needs task or other assignments.
QUALIFICATIONS: Effective oral and written skills combined with good collaborative skills are necessary to work with both the FamilyWorks team and clients. The position requires the ability to follow and comprehend program instructions and materials and to write effective case notes and correspondence. Intermediate level skills in MS Office and Outlook are required. Proven creative abilities, strong organizational and time management skills are required. The position requires the ability to travel within the City and its suburban areas.
Effective problem-solving abilities require those for analysis and summarization of information. Personal characteristics include inter-personal skills based on excellent listening skills, and the ability to collaborate and to build relationships. The role requires respect for diversity.

EDUCATION REQUIRED: A Bachelor’s degree or a progress toward completion of a degree is required.
CERTIFICATION(s) REQUIRED: None required upon entry to the position.
WORK ENVIRONMENT: The position works in an office environment, inclusive of clients’ homes. While performing the duties of this job, the FamilyWorks Coach is regularly required to sit for long periods of time, operate a computer, complete paperwork, and move about the office environment/home. It may be necessary to attend meetings/trainings off the work premises. The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The noise level in the work environment is usually moderate. The FamilyWorks Coach works in a group setting with other FamilyWorks and other YWCA staff in a shared office. The employee often interacts with members of the general public. While performing the duties of this job, the employee is frequently required to walk, stand, sit, reach with hands and arms, talk and hear. The employee must occasionally lift and/or move up to 20 pounds. The position requires some evenings and weekends. WORK SCHEDULE: Monday – Friday, 9:00am – 5:00pm. This position will require work on some evenings and weekends. Schedule can vary depending on the needs of the agency.
