About the Job
We’re excited to welcome a mid-level provider (Nurse Practitioner or Physician Assistant) to our dedicated team, providing exceptional primary care in our outpatient offices across Connecticut.
Key Responsibilities:

- Conduct comprehensive examinations, gather medical histories, and manage acute and chronic conditions.
- Order, perform, and interpret diagnostic tests.
- Develop treatment plans collaboratively with patients and their families.
- Facilitate referrals to appropriate specialists as needed.
- Work with management and staff to enhance care delivery and patient services.
- Collaborate with the care team to address care gaps and elevate quality measures.
- Engage in Patient-Centered Medical Home (PCMH) and Accountable Care Organization (ACO) initiatives.
- Ensure compliance with billing and coding requirements in partnership with practice management.
- Provide on-call coverage for the primary care group, approximately once every eight weeks.
Qualifications:
- Graduation from an accredited APRN or PA program.
- Valid, unrestricted license to practice in Connecticut.
- Current DEA registration and Controlled Substance Certificate.
- At least 1 year of experience in primary care is required.
