Seeking a part time(0.7FTE) to full time(1.0FTE) Family Medicinephysician to work with collaborative and compassionate group of Caregivers.

Join a clinic utilizing a medical home model of care working directly with a group of clinicians to provide collaborative, evidence-based care while working in a medical clinic with clinicians in family medicine and internal medicine. On-site lab and X-ray.

• CME: up to $3,300 per calendar year for CME and/or professional dues/fees (based on 1.0 FTE; prorated for lower FTE)

Requirements:

• M.D. or D.O. or foreign equivalent degree
• Successful completion of an ACGME or AOA-accredited U.S. Family Medicine Residency or RCPSC-accredited Canadian Family Medicine Residency
• Unrestricted license to practice medicine in Oregon (or ability to obtain unrestricted license to practice medicine in Oregon prior to hire)
