GENERAL DESCRIPTION: UNION POSITION
The Nurse Practitioner role at Central City Health is working with the clinical team in providing the best patient care and an excellent place to work in order to keep our patient healthy and out of the hospital. The Nurse Practitioner will practice primary care in our clinics alongside their provider colleagues.

The Nurse Practitioner will practice primary care in our clinics alongside their provider colleagues.
RESPONSIBILITIES:
- Ensures all actions, job performance, personal conduct and communications represent the organization in a highly professional manner.
- Performs comprehensive physical examinations and health assessments.
- Analyzes and interprets patient histories, symptoms, physical findings, and/or diagnostic information to assign appropriate diagnoses; consults with clinic physician and/or psychiatrist as needed.
- Advises patients, parents or guardians concerning, diet, activity, hygiene and disease prevention; provides health and wellness advice to patients/families.
- Develops treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
- Manages overall patient health through both acute illness or injury and chronic disease; provides necessary specialty care referrals as clinically appropriate.
- Effectively communicates with other health care providers or community resources to provide comprehensive, all-encompassing care.
- Educates patients about self-management of acute or chronic illnesses.
- Prescribes appropriate medications, dosages, routes, and frequencies based on efficacy, safety and cost as legally authorized.
- Counsels' patients about drug regiments and possible side effects or interactions with other substances such as over- the -counter and herbal remedies and illicit drugs.
- Detects and responds to adverse drug reactions with special attention to vulnerable populations.
- Orders, performs, and interprets the results of labs/diagnostic tests as appropriate for patient diagnosis.
- Recommends interventions to modify behaviors associated with health risks; collaborates with the patient to determine meaningful treatment goals.
- Educates and guides patients regarding disease promotion throughout the aging process.
- Emphasizes appropriate preventative care screenings based on patient age, sex, race etc.
- Provides health counseling and guidance individually or in group settings.
- Assists with the workflow of the clinic as needed, including registration and triage of patients.
- Provides quality care to patients by following the most current evidence-based guidelines and procedures of practice.
- Maintains compliance with productivity requirements ensuring timely completion of documentation (within 24 hours of the kept appointment) and thorough and accurate coding of patient visits.
- Participates in a multidisciplinary care team and identifies problems and provides feedback to team members regarding a solution; contributes to the integration of services including primary care, behavior health, dental services and supportive housing; serves as a resource to other providers and staff.
- Communicates clearly, concisely and accurately, creating a working climate that provides growth and job satisfaction.
- Demonstrates compliance with all policies, procedures and state/federal regulations; participates in the development of agency policies and programs to assure FQHC, NCQA, and other regulatory standards.
- Ensures continuous flow of communication with the leadership team to enhance the quality of healthcare service to our patients.
- Takes part in performance improvement activities such as medical staff team huddles, quality assurance audits, continuing education, Patient Centered Medical Home processes, medication administration reviews, and peer reviews.
- Participates in meetings/conferences and continuing education and demonstrates a genuine desire to develop strategies for improved clinic operations and high-level patient care.
- Ensures state licensure and national certification remain current and valid.
- Maintains economical and efficient patient care in a clean and safe environment.
- Assures patients’ rights are being maintained at all times.
- Performs other duties as assigned.
EDUCATION AND EXPERIENCE:
- Master’s degree in Nursing required
- Valid Michigan licensure as a Certified Nurse Practitioner required
- 1 to 2 years of nursing experience in a public health/integrated health setting or primary care preferred
- Experience working with patients exhibiting severe and persistent mental illness preferred
- Basic Life Support certification required; ACLS preferred
- Must meet all Continuing Education requirements
- Must be able to work cooperatively with staff and others involved in the delivery of service to patients
- Must be comfortable with community outreach and working with patients exhibiting severe and persistent mental illness
- Experience in managing multiple tasks simultaneously and meeting critical deadlines
- Must maintain confidentiality relating to treatment in accordance with HIPAA guidelines
