A health services organization in Hartford County is actively seeking a family nurse practitioner for a promising opportunity at one of their outpatient facilities.

About the Opportunity:

• Schedule: Monday to Friday
• Hours: 8am to 4:30pm

Responsibilities:

The Family Nurse Practitioner (NP) will:

• Assess, diagnose, and treat patients for both acute and chronic conditions
• Conduct patient histories, perform physical exams, and order necessary tests to inform treatment decisions
• Document patient encounters thoroughly and accurately in electronic medical records (EMR)
• Educate patients on preventive care and health maintenance strategies
• Complete documentation and charting in a timely manner to meet medical records standards and support billing requirements
• Perform other duties as needed

Qualifications:

• Connecticut Family Nurse Practitioner license
