Community Bridges is looking for 1 full-time Family Strengthening and Support Specialist!
We are a non-profit agency founded in 1982 with a mission to advance the integration, growth, and interdependence of people with developmental disabilities within their own communities. Building on the strengths of over 1,100 individuals in the greater Capital Area and their families, we work to identify barriers of access to find creative solutions to develop meaningful community engagement.
The Family Strengthening and Support Specialist will provide direct assistance to families and/or individuals ages 3 through the lifespan. The Family Strengthening and Support Specialist will utilize Person Centered approaches when working with individuals and families to help determine short and long term goals. This role will work collaboratively with the other members of Family Support Team and other in office department teams to ensure individual needs are met, families are provided with educational opportunities and connections are formed to various community agencies.
Qualifications:
Education: Associate's Degree in Human Services or related field; Bachelor's Degree preferred.
Experience: Two years' experience in human services, developmental services, special education or a related field.
Compensation and Benefits
There is a $1000 sign-on bonus for this position - $500 will be paid at the time of hire and $500 will be paid after 6 months in the role. *Please note, sign-on bonuses are for new hires only. Re-hires and transfers are not eligible at this time.
Paid Time Off: For a full-time employee working 40 hours a week, 264.16 hours of Paid Time Off is received per year! PTO days will increase over the years based on longevity and there is no waiting period to start using your PTO!

Specific Duties and Responsibilities:
- Engages families in the process of person-centered planning to clarify short and long-term goals specific to the individual's needs.
- Provides connections to community resources and educational opportunities for individuals and families.
- Collaborates with Local Education Agencies when needed for those who are under the age of 22.
- Collaborates with local community agencies as needed with individuals and families.
- Provides ongoing support to individuals over the age of 22 who do not receive waiver funded services.
- Provides all support and advocacy in a manner which takes into consideration the preferences of families and individuals and which actively encourages choice making and self-determination by the family and individual.
- Stays aware and shares resources and referral processes pertinent to individuals and families.
- Provides all support and advocacy in a manner which takes into consideration the preferences of families and individuals and which actively encourages choice making and self-determination.
- AWARDS database is updated appropriately based upon billing status of individuals on caseload. Completion of Basic Service Agreements, Transition related documents, and Person Centered Planning is completed on an annual basis as needed.
- Provides caseload coverage for the Family Support Team in the event that someone is out on leave or there is a vacant position within the team.
- Other duties as assigned.

- Work environment- Must be able to maintain a positive and professional demeanor in a busy program environment. Must exhibit high levels of professionalism.
- Physical demands: Must be able to sit or stand at a workstation. Walking is required.
