About the Job
A health services network in Long Island is currently seeking a new Supervisor to join their growing Health and Recovery Plan (HARP) team.
Responsibilities
The Supervisor will:
- Directly supervise care managers to develop effective clinical interventions and care plans.
- Ensure staff are informed of Health Home policies, procedures, and departmental priorities; maintain clinical standards through appropriate training.
- Monitor Health Home compliance and quality assurance.
- Provide crisis intervention to clients in distress.
- Complete enrollment and service date tracking reports monthly or as needed.
- Review and approve documentation as assigned.
- Exhibit teamwork, open book management, advocacy, and customer service values consistently.
- Participate in quality assurance and quality improvement activities.
- Conduct staff meetings, trainings, and supervision.
- Perform extensive fieldwork to achieve client outcomes and meet Health Home requirements.
- Facilitate bi-weekly Clinical Case Reviews with care managers.
- Perform other duties as requested.
Qualifications:
- Education: Bachelor's Degree required; Master's Degree preferred in Social Work, Psychology, or Human Services.
Preferred Qualifications:
- MSW/LMSW/LCSW.
- Two (2) years of experience working with severe mental illness population.
- One (1) year of supervisory experience.
Skills and/or Experience Required:
- Two (2) years of post-graduate experience working with individuals or families with a history of mental illness, substance use, homelessness, or HIV/AIDS.
- Excellent problem-solving, decision-making, and judgment skills.
- Ability to work effectively on an interdisciplinary team.
- Strong interpersonal and time management skills.
- Demonstrated leadership ability.
