**Position Summary:**  

We are seeking a dedicated and compassionate **Hospice RN Case Manager** to join our team. As a key member of our interdisciplinary group, you will provide comprehensive care to hospice patients in their homes and skilled nursing facilities. You will coordinate with healthcare professionals and provide emotional support to patients and families, ensuring that each patient’s final days are lived with comfort and dignity.

**Key Responsibilities:**

- Provide direct patient care, assess patient needs, and develop individualized care plans.

- Travel between patient homes or facilities, ensuring timely care and check-ins.

- Communicate with patients, families, and the interdisciplinary team to ensure continuity and clarity of care.

- Educate families and caregivers on patient conditions and care procedures.

- Maintain accurate and timely documentation of patient care.

- Provide emotional support and compassionate care for both patients and their loved ones during difficult times.

- Ensure compliance with hospice care standards, policies, and procedures.

**Qualifications:**

- Must be a licensed **Registered Nurse (RN)** in the state of Texas.

- Minimum of 1-2 years of experience in hospice, palliative care, or a related field preferred.

- Strong communication skills and the ability to work with diverse patient and family populations.

- Must have a valid driver’s license and reliable transportation, as travel between patients is required.

- A compassionate and caring heart, with a commitment to follow-through on patient care.

- Ability to manage time efficiently and work independently.
