Job Summary:
The Patient Navigator builds working relationships, solves problems and supports patients while they learn to self-navigate the healthcare system as it relates to the patient's diabetes. The Navigator directs patients to resources and manages information. The Navigator is knowledgeable and a creative problem solver. They are responsible for staying informed of changes in the healthcare organization, seeking information about available patient resources, working with members of the healthcare team to facilitate patient care, promoting patient health and comfort, and supporting patients while they learn to self-navigate.
Job Responsibilities:
- Manage and track information i.e. patient profiles, appointments and interactions
- Maintain patient confidentiality
- Help patients "navigate" the maze of clinics, offices, administrative systems, and patient support services
- Reduce barriers that keep patients from getting timely treatment by identifying patient needs and directing them to sources of emotional, financial, administrative or cultural support
- Assist patient and family with setting up systems necessary for telehealth care of diabetes as well as the technology necessary for self-care of diabetes i.e. pumps, sensors and meters
- Assist patient and family with sharing information related to patient's diabetes remotely with the Diabetes team
- Assist with daily functions of the clinical and research activities of the Naomi Berrie Diabetes Center when necessary
- Other duties as assigned
Job Qualifications:
- High School Diploma or GED equivalency required
- At least one year of experience in a medical office environment
- At least one additional year of experience in a patient or customer facing position
- Ability to follow through and handle multiple tasks simultaneously
- Knowledge of electronic medical records (EMR) system or EPIC is preferred
- Bilingual in Spanish is preferred
