About the Job
JOB REQUIREMENTS: The Utilization Review and Health Information
Management (HIM) Specialist is responsible for coordinating insurance
reviews and clinical updates for the inpatient psychiatric hospital at
the Community Treatment Center along with clerical duties and support
functions such as scanning medical records, assembly of medical records,
analyzing medical records for compliance with documentation
requirements, requesting medical records, transcription of medical
dictation, and clerical support for the providers. Essential Duties
Conducts quality medical records reviews and compiles records to forward
to insurance companies to support continued hospitalization, treatment,
and/or care. Tracks insurance authorizations, last covered days (LCD)
and ensures compliance with any timeframes associated with extending
adequate coverage. Consults with Interdisciplinary team members and/or
Attends treatment plan meetings, when necessary, to gain collateral
information to forward to insurance companies. Monitors patient length
of stay and extensions and informs clinical staff of issues that may
impact length of stay. Serves as liaison responsible for establishing
and maintaining effective interactive communication between the
clinicians, multi-disciplinary clinical staff members and Utilization
Review Committees/Quality Assurance Committees. Communicates effectively
with Admissions Clerks and Billing Department Personnel to coordinate
the patient's coverage. Facilitates peer to peer review calls between
facility and insurance providers to support the medical necessity of
continued treatment. Initiates and completes the formal appeal process
for denied admissions or continued stays. Scans paper medical records to
electronic health record. Performs quality checks to assure records are
scanned to the correct client/resident under the correct form type and
episode of care. Assembles paper medical records and checks that proper
identification is on each form. Reviews medical records of discharged
clients/residents for record completeness. This is a comprehensive
analysis of the content of the medical record. Prepares chart deficiency
slips and routes to providers for dictation and signatures. Requests
medical records from other organizations, as needed. Transcribes
medical/psychiatric dictation accurately and efficiently to provide a
permanent record of client/resident care. Demonstrates an understanding
of the medicolegal implications and responsibilities related to the
transcription of client/resident records to protect the
clients/residents and Brown County Community Treatment Center. Makes
copies of transcribed reports and distributes as applicable. Types and
revises medical record forms and policies and procedures submitted by
leadership staff. Assists with other HIM/clerical duties as needed.
Follows policies and procedures to contribute to the efficiency of the
Health Information ManagementDepartment. Understands and applies
confidentiality standards. Completes and maintains necessary
documentation according to applicable requirements and policies.
NON-ESSENTIAL DUTIES: Performs related functions as assigned. MATERIALS
AND EQUIPMENT USED: General office equipment Digital dictation equipment
Computer PHYSICAL DEMANDS: Lifting 20 pounds maximum with frequent
lifting and/or carrying o objects weighing up to 10 pounds. Intermittent
standing, walking and sitting. Using hands(s)/feet for repetitive single
grasping, fine manipulation, pushing and pulling, and operating
controls. Occasional bending, twisting, squatting, climbing, reaching,
and grappling. Communicating orally in a clear manner. Distinguishing
sounds at various frequencies and volumes. Distinguishing people and
objects at varied distances under a variety of light conditions. Must be
free from communicable disease. * OTHER EXPERIENCE AND
QUALIFICATIONS: Knowledge of general office procedures. Knowledge of
En lish, grammar, spelling, punctuation and style. Knowledge of medical
terminology, psychiatric terminology, anatomy and physiology and
pharmacology. Knowledge of and ability to utilize a computer and the
required software. Skill in transcribing complex material and
understanding diverse accents and dialects and varying dictation styles.
Ability to type at a minimum rate of 60 net words per minute. Ability to
transcribe information from dictating and transcribing equipment.
Ability to learn and apply the specialized knowledge and terminology of
the Health Information Management Department and to use designed
reference materials. Ability to follow oral and written instructions;
excellent listening skills. Ability to communicate effectively both
orally and in writing. Ability to accept responsibility. Ability to
establish and maintain effective working relationships with staff and
the public. Ability to maintain the confidential nature of client and
business information. Ability to work independently with minimal
supervision. Ability to work under pressure with time constraints.
Ability to concentrate. Ability to work the required hours of the
