Cabell Huntington Hospital is seeking a full-time Heart and Stroke Program Coordinator. The Coordinator allocates human resources to promote efficient, effective, safe and compassionate nursing care based on current standards, practice, and evidence-based guidelines. The Coordinator promotes shared decision-making and professional autonomy by providing input into unit-level decisions concerning the care of heart failure and stroke patient and keeping the staff informed. They problem solve to prevent a potential crisis, assess, evaluate, and manage unit operational issues that pertain to care of the heart failure/stroke/cerebrovascular disease patient using a multidisciplinary team approach. The Coordinator is involved in the planning and implementation of community screening events and outreach programs to further the development of the Heart Failure and Stroke Program. The coordinator has responsibility for tracking and documenting outcome data for heart failure, stroke and co-morbidities and post-hospitalization follow-up information / outcome data; responsibility to improve outcomes this includes conducting meetings for heart failure, stroke and co-morbidities. Be an active participant in local, state, and national organizations related to disease specific initiatives, education, and process changes. Meet all requirements for desired level of West Virginia Office of Emergency Medical Service Stroke Center Categorization and preparation of verification visits. Meet all requirements of Joint Commission of Hospital Accreditation Stroke Certification and maintain preparation of verification visits. Meet CMS core measures. Meet all requirements of the American College of Cardiology Accreditation Services. The Coordinator assists with education and training based on the staff’s needs while making sure that clinical competencies of staff meet standards of practice in the care of heart failure and stroke patient. The Coordinator facilitates an atmosphere of interactive management and the development of collegial relationships among nursing personnel and others. The Coordinator serves as a link in continuity of care between nursing personnel and other healthcare disciplines and shall facilitate open communication to support the continuum of care for the patient with heart failure and stroke/cerebrovascular disease. The Coordinator is responsible for monitoring Performance improvement indicators for Heart Failure and Stroke and involving staff in findings and resolutions. The Coordinator assures that care is delivered with respect for patient rights. The Coordinator assists in creating an environment that is open and respectful and promotes the sharing of expertise to promote the benefits of health outcomes. The Coordinator contributes to the unit processes, day-to-day operations, standards of care, and attainment of goals of the assigned unit. The Coordinator evaluates personal performance based on professional practice standards, relevant statutes, rules and regulations, and organizational criteria. The Coordinator collaborates with nursing leadership, Director of Nursing and SMMC/MH cardiac centers of excellence leadership in planning, innovation, and evaluation of activities. * Graduated from Accredited School of Nursing required, Master's Degree preferred. * Current Active unencumbered License in West Virginia required. * AHA BLS required. * Specialty Certification (neurology, stroke, heart failure) preferred. * 2 years of position-related experience required.
