Summary
Assists clients with medication reminders, dressings, transfers, light housekeeping, light meal preparation, and other services as needed. Offers companionship to support and engage clients. Escorts to and from the dining room and activities as needed. Assists clients toward maintaining independence thus aiding in reducing attrition within assigned Senior Living Buildings. Assists clients in adjustment to new home environment. Welcomes new residents to the building and promotes Senior Support Solutions services. Meets with residents recently home from the hospital or rehab to ensure their wellbeing and promote Senior Support Solutions services. Provides clients comfort and safety. Actively monitors health and wellbeing of clients. Informs the Clinical Care Manager of changes in clients' conditions and needs. Composes a shift report at the end of each shift detailing necessary information and emailing it to Clinical Manager and Operations Manager. Completes and submits documentation as required. Reads email updates at the start of each shift. Reviews and prints current Service Chart at the start of each shift. At the end of the shift, faxes Service Chart to Senior Support Solutions Office. Performs other job-related duties as assigned including assisting in Senior Living Communities with residents who may not be a client of Senior Support Solutions. May be assigned other duties as appropriate.
Job Qualifications
- High school diploma or GED
- Certified Nurse Assistant preferred, but not required
- Previous experience in customer service required
- Previous experience in Senior Living Communities strongly preferred
- Critical thinking skills
- Ability to travel between communities
