Position Summary:
Under the direction and supervision of the New Beginnings In Home Services Manager and Staffing Coordinator is responsible for providing and assisting participants with activities of daily living, restorative and supportive care necessary to preservation of the home environment. Assists in monitoring the health status of and providing a safe environment for participants. Functions as a member of the multidisciplinary team.
Home Health Aide responsibilities include:
Providing health care services in patients residences
Performing domestic and household tasks
Transporting and accompanying patients to doctors office or to hospital
Responsibilities
· Provide health care services in patients residences
· Perform domestic and household tasks
· Transport and accompany patients to doctors office or to hospital
· Administer simple prescribed medications
· Assist with clients personal care activities
· Monitor patients (vital signs, temperature, respiration, etc) and report on their condition
· Maintain patients care records and document provided services
· Assist patients with mobility and physical therapies/exercises
· Instruct and counsel patients and families on diet and exercise
· Collect routine specimens
· Provide companionship and basic emotional or psychological support
Requirements and skills
· Proven working experience as Home Health Aide
· Ability to monitor vital signs and to collect specimens
· Familiarity with basic nutrition and personal hygiene standards
· Proficiency in English
· Nursing and health care administration skills
· Caring and compassionate personality
· Current CPR certificate
· First Aid training
· Valid current driver’s license and auto insurance and reliable transportation
· High school degree
· Must be 18 years or older
Physical Requirements and Working Demands
· Requires the ability to lift/carry up to 65 pounds and push or pull objects exceeding 150 pounds, using appropriate body mechanics. Requires intermittent driving thoughout day, between participants/client’s homes.
· Requires full range of body motion including handling and lifting patients, manual and finger dexterity and eye-hand coordination. Requires standing, walking, pushing, bending kneeling and reaching at arm’s length and overhead in community for prolonged periods of time. Must be able to transfer participants needing total assist.
· Must be able to make home visits to participants who live in non-handicapped accessible dwellings.
· May be exposed to a risk of bodily injury through contact with moving instrumentation, toxic substances, medicinal preparations, bodily fluids, cleaning supplies, communicable diseases and any other conditions common in a home environment. Subject to unpleasant odors and living conditions of participants. Maybe subjected to adverse driving conditions including wet, icy, slushy and snow packed roads.
