POSITION SUMMARY:
- Performs Bathing and ADL assistance to clients in their homes in accordance with Medicare, Arizona, and Federal regulations and guidelines.
- Contributes and participates in care coordination and patient-centered care as a part of an interdisciplinary clinical care team of nurses, therapists, CNA's and MSW's.

QUALIFICATIONS:
- Arizona CNA License.
- Minimum 6 months of experience; recent experience preferred, but not required.
- Arizona Driver’s License.
- Vehicle to travel to the patient location and proof of auto insurance.
- Level-one Fingerprint Clearance Card.
- TB test and clearance.
- Current CPR certification.
- Pre-employment drug screen
- Ability to perform job description and work without restrictions.
