Description: Our client is currently seeking a Home Health LPN/LVN
This job will have the following responsibilities:
•Implement the plan of care initiated by the registered nurse.
•Provide accurate and timely documentation consistent with the plan of care.
•Assess and provide patient and family/caregiver education and information.
•Participate in coordination of home health services.
•Use equipment and supplies effectively and efficiently.
•Participate in personal and professional growth and development.
•Perform other duties as assigned by the registered nurse.
•Record a patient’s medical history accurately.
•Take and record measurements of blood pressure, temperature, heart rate etc.
•Observe patients under treatment to identify progress, side-effects of medications etc.
•Monitor patients’ condition including fluid intake and output and compose patient charts.
•Assist ailing patients in daily necessary activities such as eating, bathing etc.
•Provide emotional and psychological support when needed.
•Communicate with patients’ family or friends to provide advice, comfort, and release instructions.
Qualifications & Requirements:
Qualifications & Requirements:
•Graduate of an accredited practical nurse or vocational nursing program.
•Minimum of one (1) year of experience (per state regulations) preferably in home care or med/surge and shall work under supervision of an RN.
•Current licensure in state and CPR/BLS certificate.
•Complies with accepted professional standards and practice.
•Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
•Reliable transportation to get to patients and the office for mandatory meetings.
