Principal Responsibilities:
The physical therapist is responsible for performing treatments and exercises to aid patients in recovering from illnesses and injuries. Their primary duties include using various techniques to develop personalized treatment plans, prescribing medications to relieve pain and increase mobility, and teaching patients exercise techniques to improve their health. This position reports to the Home Health Director and involves the following responsibilities:
- Providing physician-prescribed physical therapy
- Diagnosing movement dysfunction by listening to patients and observing their movement
- Developing individualized treatment plans with all other home care personnel in patient care planning
- Consulting with physicians regarding changes in treatment
- Writing reports to physicians regarding patient progress
- Instructing patients and their families/significant others in-home programs and activities of daily living
- Participating in in-service programs and presenting in-service programs as assigned
- Participating in Quality Assurance and Performance Improvement activities as assigned
- Attending all patient care conferences as scheduled
- Preparing clinical and progress notes
- Assisting physicians with evaluating the level of function
- Helping develop the plan of care and revising it as necessary
- Consulting with family and agency personnel
- Completing and submitting OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition per Agency-defined time frames
- Appropriately utilizing ICD-10 codes
- Utilizing standardized assessments and evidence-based practice to support clinical interventions
- Evaluating treatment outcomes for effectiveness and modifying services concerning patient evaluation and making referrals as necessary
- Providing consultation and counseling to patients, families, caregivers, and other service providers related to physical orders
- Effectively communicating with the Home Health Director, physicians, and other health team members regarding patient progress, barriers, and treatment plans
- Supervising and supporting physical therapist assistants, rehabilitation aides, and/or students
- Adhering to company-required policies and procedures and code of conduct
- Having the ability to exercise initiative and independent judgment
Qualifications:
- Graduate from a physical therapist curriculum approved by the Commission on Accreditation in Physical Therapy Education, American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association in the state(s) in which practicing
- Acceptance of the philosophy and goals of this agency
- Active state licensure in the state(s) in which practicing
