Home Health Registered Nurse

One of our top clients is seeking a dedicated and experienced Home Health Registered Nurse (RN) to join our growing team. In this rewarding role, you will provide high-quality nursing care to patients in the comfort of their own homes, promoting independence and well-being.

Home Health Registered Nurse

Responsibilities:

• Conduct comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care, including medication administration, wound care, and chronic disease management (no skilled nursing component).
• Educate patients and families on self-care and disease management strategies.
• Collaborate with physicians, therapists, and other healthcare professionals to ensure coordinated care.
• Document care diligently and accurately in electronic health records.
• Maintain a safe and positive work environment for patients and yourself.
• Travel to patients' homes throughout [City/County] area (valid driver's license and reliable transportation required).

Qualifications:

• Active and unrestricted Registered Nurse (RN) license in the state of Georgia.
• Minimum of 2 years of experience in home health care (skilled nursing experience not required).
• Strong assessment, critical thinking, and problem-solving skills.
• Excellent communication, collaboration, and interpersonal skills.
• Compassionate and patient-centered approach to care.
• Proficient in documentation and EHR systems.
• Ability to work independently and manage time effectively.
