SpiriTrust Lutheran is now hiring a Full-Time Day Clinical Supervisor for our Home Care & Hospice Program in Franklin County! Must be a Registered Nurse.
Required: Graduate of a school of nursing/therapy with a current license(s) to practice as a nurse/therapist in the state(s) served by SpiriTrust Lutheran Home Care & Hospice. Current CPR certification within three (3) months of employment.
Experience: A minimum of two (2) years of experience in a community health agency normally required. A minimum of two (2) years of supervisory experience in community or public health agency normally required.
General Responsibilities:
- Responsible for coordinating care provided to designated clients
- Supervises client and agency activities
- Participates with the Regional Manager in establishing specific goals for the department
- Coordinates work within the department, as well as with other departments
- Ensures compliance with operating policies and procedures and outside regulatory requirements
- Directly supervises assigned personnel
- Reviews all available client information from Admission Care and assists with the assignment of the RN, therapist and appropriate home health aides
- Discusses potential clients with core members of the Interdisciplinary Group, attending physician and/or the Hospice Medical Director to confirm prognosis and their agreement with enrollment in a specific benefit.
- Receives written and verbal reports from each team member including assessment reports from RNs and therapists.
- Assists in identifying client needs and recommends appropriate intra/inter agency referrals.
- Responsible for monitoring case management assignments according to agency procedure.
- Assures appropriate orders for all visits and care.
- Tracks hospitalized clients and assures scheduling of proper follow-up visits.
- Assists in the development of the plan of care in consultation with the assessing staff.
- Interprets and implements agency policies, governmental regulations, standards of nursing practice and reimbursement requirements as related to direct care.
