Position Overview:
- The Hospice Community Liaison is responsible for establishing and maintaining public relations at facilities, hospitals, and other community businesses.
- The candidate must be self motivated and know how to provide hospice education.
- The Hospice Community Liaison is responsible for the process from referral to admission, which includes meeting with the patient and family and giving an overview of hospice service and obtains consents for care.
- Concho Hearts Hospice is dedicated to providing compassionate care wherever you call home.
- Our team of Nurses, Aides, Chaplains, Social Workers, Volunteers, and Doctors provide symptom management, and patient and family support while coping with an end-of-life illness.

Responsibilities:
- Develops various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
- Consistently visits, communicates and monitors provider, community, customer, payer, and patient perceptions of Concho Hearts Hospice as a high-quality provider of services.
- Conduct educational in-services and community outreach to promote our superior hospice care services.
- Educates patients and their families about hospice expectations.
- Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to a home.
Qualifications: MUST have Hospice Marketing experience of 2+ years.
