Job Description:
Seeking a dedicated and detail-oriented Hospice Patient Care Manager to join our team. This office-based position will be responsible for overseeing key administrative functions that ensure the smooth operation of our hospice services. The ideal candidate will have experience in healthcare or hospice environments, with a strong understanding of patient care coordination and administrative processes.
Key Responsibilities:
- Screen and interview new candidates for CNA and Hospice Aide positions.
- Reconcile invoices related to patient services and staff activities.
- Assign patients to staff members, ensuring appropriate care coordination.
- Manage medical records in compliance with hospice care standards.
- Handle referral benefit verifications to ensure proper patient services.
- Conduct data entry and maintain patient and staff records.
- Prepare for IDT meetings by organizing patient care information.
Qualifications:
- Experience in hospice, healthcare, or administrative roles.
- Strong organizational skills and attention to detail.
- Excellent communication and interpersonal skills.
- Familiarity with electronic medical records (EMR) systems.
