What You'll Do:
As a Hospice RN, you will carry out our Mission of Love by assuring quality care of our Hospice patients in a caring, timely and appropriate manner, paying particular attention to management of patient's related symptoms and comfort level.
Our Hospice RN is responsible for the following:
- Maintaining a working knowledge of standards of care for hospice programs in order to insure compliance for Medicare certification and state licensure of the hospice program. Completing documentation including assessments, certifications and care plans in accordance with the Medicare Conditions of Participation for hospice programs.
- Attending and participating in IDG, mandatory staff meetings, and in-service programs.
- Maintaining communication and providing updates or changes in patient condition to the patient's family, primary care physician, facility staff and hospice medical director.
- Monitoring quality end-of-life care by routinely assessing patient's needs. Completing accurate documentation, and coordinating care with other members of IDG. Providing end-of-life education for patient, family members, caregivers and friends; responding to the physical, mental, social and emotional needs of terminally ill patient and their loved ones.
- Revising/updating care plans and performing assessments of family/caregiver needs on an ongoing basis.
- Pronouncing death in the absence of a physician and making all appropriate notifications. Completing all required documentation.
- Assessing, administering and documenting professional nursing care to assigned Hospice patients utilizing the nursing process in accordance with both the nursing and administrative policies.
- Performing patient care responsibilities in an environment that optimizes patient/caregiver safety and reduces likelihood of medication/healthcare errors. Demonstrating knowledge of medications and their correct administration. Coordinating all patient services. Assessing pain and utilizing appropriate pain management. Offering support and palliative care.
What You Have:
- Current RN licensure by the Pennsylvania State Board of Nursing.
- CPR certification prior to or upon employment.
- Previous hospice experience.
- Valid PA driver's license and insurance on personal vehicle.
