Responsibilities:
Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
Collaborate with leadership to review care coordination initiatives
Develop intervention plans, and share ideas about network management issue
Work alongside pharmacists on medication management
Complete Medicare required visits
Complete acute, follow up, and discharge visits as necessary
Implement an individualized plan of care for patients' goals
Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
Facilitate and assist leadership in quality improvement initiatives and risk management
Timely response in chart completions and pharmacy recommendations
Provide oversight and mentorship to our clinicians in the greater Chicago, IL area (Glenwood, IL)
Experience as a medical director in a skilled nursing facility preferred. However, clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), geriatric, primary care, internal (IM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM).
Our compensation is a productivity-based model (Fee for Service) that offers you control to maximize your income potential. Expected compensation is estimated range of $76,000 to $257,00 annually with no cap on productivity income potential.
