Housing Specialist Position Summary:
The Housing Counselor, collaborates with Health Homes to locate homeless or unstably housed Health Home members and provides counseling and housing assistance to members in the program who are enrolled in New York State’s Health Home Program. By using identified best practices, procedures and methods, the Housing Counselor provides housing as a means to facilitate access to health services and improve the health status of Health Home members and to coordinate effects of the Health Home Care Manager and Housing Counselor to implement the Health Home Member’s Plan of care.
Housing Specialist Job Responsibilities:
- Provides ongoing short term rental and emergency rental or utility assistance for Medicaid members enrolled in New York State’s Health Home Program who are either homeless or housing vulnerable.
- Completes assessment of needs related to maintaining permanent housing.
- Establishes individualized care plans and implements goals in cooperation with Health Home Care Managers and other members of a multi-disciplinary team. Assures continuity of member services.
- Responds to and effectively handles emergency situations which may require flexibility of time and area of assignment. Shares on call responsibility and takes necessary action to ensure staff coverage, appropriate crisis interventions, and untoward incident notification.
- Establishes and maintains effective relationships with other service providers such as DSS, SSI, HUD, Section 8, Shelter Care Plus, Landlords, and Care Managers.
- Completes all required documentation, in timelines in accordance with program standards.
Housing Specialist Qualifications:
HS Diploma and two (2) years’ related experience required. Bachelor’s or Associate’s and/or related experience preferred. Must possess a valid New York State Driver’s License and access to reliable transportation.
Starting Rate: $20.19 per hour
