About the Job
- Technical Support: Provide technical assistance and guidance during the integration process. Troubleshoot and resolve issues to ensure smooth and successful integration.
- Monitoring and Follow-Up: Regularly monitor clients’ progress and service utilization. Provide feedback and coaching to clients as needed.
- Documentation: Maintain accurate and up-to-date case records.
- Communication: Facilitate effective communication between clients, the internal team, and providers. Ensure all parties are informed and involved in the integration process.
Skills:
- Strong organizational and time management abilities.
- Excellent analytical and problem-solving skills.
- Capability to work independently and collaboratively within a team.
Integration Tasks:
- Set up physician and note categories.
- Coach staff on utilizing the integration for consultations and notes.
- Follow up on patients not mapped through the integration.
- Request deactivations for facilities that are no longer serviced or have changed ownership.
- Troubleshoot support cases
- Provide activation instructions for pending enrollments.
Experience:
- Preferred Experience working with Skilled Long-Term Care facilities.
- Intermediate proficiency with PC and Windows applications
- Proficiency in software such as Microsoft Office and Excel is required.
- Preferred experience with Point Click Care software and Matrix EHR software.
