Provides professional nursing care and clinically-based care management according to specified physician orders and an Interdisciplinary Team (IDT), and in conformance with established nursing techniques, procedures, standards and adherence with state and federal regulations governing a Program of All-inclusive Care for the Elderly (PACE). Responsible for assessing, coordinating and delivering care to a group of participants as assigned. In this role, the RN is responsible for developing and implementing a nursing care plan to support the Interdisciplinary Care Plan and provide treatment as directed by a Primary Care Provider and the IDT. Responsible for coordinating participant care during transfers between inpatient and community settings including discharge planning for a safe discharge, nursing assessment to determine necessary orders for care, monitoring on community care and communication with all members of the IDT. At management’s discretion, this position may be assigned to provide care in clinic, adult medical day health center or home care settings. This position is a member of the Interdisciplinary Team (IDT) as defined in PACE regulations.

$55/hr

1. As an IDT member: Conducts nursing assessments of all member's nursing care needs at enrollment, semi-annually and change of status. Conducts assessments at transfer between inpatient and community settings.  Collaborates with IDT members, the participant and participant’s caregiver/responsible party to develop a comprehensive, holistic care plan.
2. Provides nursing care and nursing care management across settings. Provides skilled care in all settings as needed. Treatment shall be provided in the location most suitable and accessible for the participant, including home, DHC and clinic as assigned and as participant needs dictate.
3. May be required to function as a delegating nurse, responsible for oversight of clinical performance of personal care aides.
4. Development in conjunction with the IDT, and ongoing review of an established emergency plan for each participant.
5. Provision of skilled nursing as requested by the IDT and ordered by a medical provider.
6. Continually assesses the participant's care needs and the effectiveness of services provided.
7. Administers medications and assists physicians in administering treatments and diagnostic tests. Monitors intravenous therapeutic treatments.
8. Makes periodic participant assessments, takes vital signs and evaluates participant care provided by LPN’s, technicians, nursing assistants, etc.
9. Reports significant changes in participant’s condition or affect to the appropriate IDT professional(s). Assists with or implements corrective measures as directed during adverse participant developments.
10. Maintains current, accurate and complete records of nursing observations and care.
11. Responds to participant inquiries, complaints/grievances and service requests. Adheres to CMS guidance in reporting and assessing service requests.
12. Attends in-service training and continuing education programs.
13. Performs related nursing duties as assigned.

Required Knowledge, Skills, and Abilities

1. Work requires the level of knowledge typically acquired through completion of a Bachelor’s degree in nursing or closely related field and three-five years of previous experience, preferably including case management, discharge planning and/or utilization review.
2. Requires licensure as a Registered Nurse. AHA BLS is required.
3. If required to function as a delegating nurse, obtain and maintain delegating nurse status according to the Maryland Department of Health guidelines
4. Must have a minimum of 12 months of experience caring for the frail elderly
5. Work requires the analytical skills to resolve problems that require the use of basic scientific, mathematical, or technical principles.
6. Work requires the ability to communicate effectively both orally and in writing and provide empathy in difficult interpersonal situations.
