About the Job
GENERAL PURPOSE OF JOB
The job coach will be able to assess and prepare the people we support in finding work in the community. They will work with the Care Manager to develop a Supported Employment Plan that will be used to direct the person’s direction in employment support. They will work with a caseload of people and employers to develop working relationships that enable them to have success in competitive employment in the community.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Job development in the community
- Job placement of appropriate candidates
- Follow the process and requirements of the Employee Training Program (ETP), when applicable.
- Provide services of at least 4-7 billable hours per day.
- Document accurately and contemporaneously for billing for SEMP, PreVoc and Day Hab services.
- Engage in Pre-Voc volunteer opportunities.
- Will attend required Eleversity trainings for SEMP.
- Interact with employers concerning people on their caseload.
- Teach units at I-SAIL academy
- Work one-on-one with people supported on their specific needs
- Provide transportation to and from hub location when needed.
- Job coach on a regular basis for people on their caseload
- Document activities on a daily basis in Therap.
- Maintain quality services at all times based on the participant’s individual valued outcomes.
- Open communication with supervisors.
EDUCATION AND/OR EXPERIENCE
- High School Diploma/GED is required; an associate degree is preferred.
- 1 year of related experience is preferred. Working as a caregiver or with adults with developmental disabilities.
- Be empathetic and have a team approach to care for the individuals
- Have excellent written and verbal communication skills
- Be energetic, creative, and have a sense of humor
DRESS CODE
Are expected to wear sturdy, closed-toe footwear that enables them to have good traction and balance. Expected to avoid clothing that is excessively tight, short or revealing. No part of under garments should be visible outside clothing. Shirts and tops should cover the entire torso. Hats/hoods and sunglasses will be removed while indoors.
SKILLS
Language Skills: The employee must have the ability to read, comprehend, and write simple instructions, short correspondence/memos. Have the ability to effectively present information in one-on-one and small group situations.
Mathematical Skills: Have the ability to add, subtract, multiply and divide 2-3 digit numbers using units of United States’ currency, weight, volume and distance measurements.
Reasoning Skills: Have the ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Have the ability to problem solve, involving several concrete variables in standardized situations.
CERTIFICATES, LICENSES, REGISTRATIONS
- Valid NYS driver license.
- The employee is required to successfully complete several agency training courses, including but not limited to crisis intervention and prevention training, Medication Administration, PR/First Aid, SCIP-R and Behavior Management. Upon hire and as required through certification standards. Employees will be required to maintain any specialized training as required by position and regulation.

While performing the duties of this position, the employee is regularly required to stand and walk. The employee is occasionally required to sit, climb, balance, stoop, kneel, crouch and crawl. The employee must be able to lift up to 50lbs.

All staff must pass all NYS Justice Center Background Check Requirements, and Physical. In addition, the employee has a duty to report any suspected violations of the law (including any motor vehicle infractions) to the Human Resources Department.
