LIVING DONOR PATIENT NAVIGATOR - (Temple University Hospital/40 hours days) - (246084)

 The Living Donor Patient Navigator works closely with the living donor nurse coordinators and support coordinators. Supporting the living donor program through and variety of tasks, assisting patients to navigate the living donor process. Represents Temple University Hospital's (TUH) Center for Living Kidney Donation in various provider and community settings throughout the region. Serves as a liaison between The Center for Living Kidney Donation and The National Kidney Registry (NKR), assuring the accuracy of reported information and assuring that the Center for Living Kidney Donation remains in compliance with our NKR participation requirements. Identifies patient-centered problems with service delivery, works with the multidisciplinary team to develop sustainable solutions to these problems and evaluates the effectiveness of these solutions from the patient and provider perspectives. Provides support for the educational and organizational components of the living kidney donor program. Completes additional duties as assigned.

Education
Bachelors Degree : Healthcare or related field (Required)
Combination of relevant education and experience may be considered in lieu of degree.

Experience
2 Years experience in a healthcare setting (Required)
Experience in Living Donation, Transplant, or related area (Required)
General Experience in healthcare education, healthcare outreach or working with transplant patients (Preferred)

License/Certifications
RN-LIC - PA Registered Nurse License (Preferred) or SW-LIC - PA Social Worker License (Preferred)
BLS - Basic Life Support (Preferred)
