ESSENTIAL FUNCTIONS Documentation 1. Documents in accordance with standards of practice and established policies and procedures of the Ranch and accrediting and regulatory standards. Consistently records client progress in relation to treatment goals. Demonstrates the ability to balance the inclusion of information with client rights to confidentiality at it pertains to the position held. 2. Complete documentation timely and appropriately for reimbursement and for alternative funding for youth and family benefits in accordance with established standards. 3. Performs clerical/administrative duties related to nursing work, medical records. 4. Initiate and develop forms, letters, and QA aides as needed. 5. Initiate admission Emergency Medical file. 6. Coordinates physicians and clinical orders and maintains medical files. 7. Delivers newly ordered medication to appropriate program site and documents in respective medical logbook. 8. Inventories and maintains supplies for nursing offices and first aid kits. Medical Assessment 9. Evaluate client and check vital signs. 10. Ensure that evaluation, observation, and documentation for utilization of restraints are in accordance with clinical administrative standards. 11. Schedule clients for consultations with staff physician, as necessary. 12. Review laboratory and related psychiatric and medical health assessments as clinically indicated. 13. Provide diagnostic reassessment at time of transfer and/or discharge at it pertains to the position held. 14. Assists in the health screening of youth by initiating the health screening examination when necessary, establishing screening and medical profile as needed, ensuring required immunizations are current and documented. 15. Provides or assists in the provision of crisis interventions through evaluation, observation, and documentation of restraints (and/or seclusion) in accordance with established policies and procedures. Medical Client Care & Follow-up 16. Provide client care and follow-up including re-evaluations within scope of position. 17. Ensure compliance of treatment with Quality Assurance standards. 18. Provide nursing and emergency care to clients and advises appropriate follow-up as it pertains to the position held. 19. Effectively conduct follow up reports regarding care. 20. Provides/coordinates in-service relative to medical issued, conditions or concerns, present in the youth (client) population or as necessary in accordance with established standards, policies, and procedures. 21. Completes rounds within the campus-based programs consulting with staff members, evaluating special precautions, as necessary. 22. Takes off physicians' orders and orders medications in accordance with prescriptions. 23. Helps in scheduling medical appointments in accordance with physicians' orders. 2 Medication Coordination and Monitoring 24. Provide medication review and monitoring, including instruction for youth and family as requested and standing order medications per policy. 25. Completes medication counts within open campus program as assigned within the Intensive Care Center. 26. Orients/trains staff to medications and "special " physical and/or medical conditions inherent in the youth's treatment. Medical Care Coordination & Education 27. Direct, consult, and coordinate medical services with programs and facility. 28. Provide staff educational in-service, training, and QA/CQI assistance as it pertains to the position held. 29. Effectively educate staff regarding treatment plans and medical conditions pertaining to clients. 30. Meet with youth families and treatment team members in accordance with treatment plans, clinical necessity, or established standards. 31. Educate team members in relationship to treatment advised by the physician. 33. Participates in the development, coordination and monitoring of medical guidelines and assists in the dissemination and training of them in accordance with the policies and procedures. 34. Participates in the development, implementation, review, and monitoring of QA/CQI standards and practices. 35. Transports clients, as necessary.
