Citizens Memorial Hospital (CMH) is recruiting a Licensed Practical Nurse (LPN,LVN) or Certified/Registered Medical Assistant (MA) to assist and triage patients over the phone when calling their primary care clinic.

Care Transitions is responsible for answering and returning patient phone calls for CMH primary care clinics. The department is made up of two teams: schedulers and triage nurses, who work closely together to make sure our patients get the best care!

A typical day will include: answering and returning patient calls for triage and nursing advice. Relays information and results between patients and their primary care provider. Assists with facilitating medication refills, authorizations and questions. Identifies and connects patients to organizational resources. Provides excellent customer service and behaves in a manner consistent with the mission of CMH. Performs other duties as requested. The ideal candidate will have experience working with patients in a clinical setting.

The position is: Part-time;Mondays are mandatory-other days we can be flexible to help give you the best fit for your needs!

No weekends, calls, or holidays!!

Job Requirements:

• Education: Associates degree, or Certified Med Assistant
• Skills: Patient care
• License: LPN, CMA,RMA
