Job description/Responsibilities: Full-Time (40hrs per week) All Shifts APPROXIMATE PAY RANGE: $38.54 - 44.73 JOB SUMMARY: The Licensed Vocational Nurse provides direct and indirect patient care services that meet the psychosocial, physical, and general aspects of care, meet the communication needs of the patient and family under the supervision of a Registered Nurse. DUTIES AND RESPONSIBILITIES 1. Consistently uses two patient identifiers when administering medications and performing procedures. 2. Consistently follows medication reconciliation policy. 3. Is in compliance with the information contained in the Infection Manual (i.e. OSHA bloodborne pathogen standards, CDC Guidelines), and demonstrates appropriate use of personal protective equipment (i.e. gloves, gowns, masks, goggles) and handwashing techniques. 4. Able to give a thorough, detailed efficient change of shift reports to ensure consistency in the plan of care. 5. Uses time out on all patients for any bedside procedures in the unit. 6. Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e. suction and oxygen immediately available when needed. 7. Consistently reports changes in patient condition immediately to the RN. 8. Completes documentation of narcotics given to the patient on the MAR and narcotic sheet. 9. Recognizes signs of abuse and reports appropriately to Adult/Child Protective Services. 10. Detects and reports medication errors and ADRs according to hospital policy. Safely and accurately administers medications and documents properly on the MAR in a timely manner. 11. Initiates discharge and patient/family education planning with proper documentation on the patient record in Medi-Tech. 12. Participates in the development of the plan of care. Evaluate and adjust the plan of care daily and communicate the plan to the Registered Nurse. 13. Establishes priorities of patient care based on essential needs and available resources of time, equipment, personnel, and supplies. 14. Formulates, maintains and updates a goal-directed plan of care on patients which is prioritized and based on patient outcomes. 15. Performs other duties as assigned or required.

Required Qualification: EDUCATION, EXPERIENCE, TRAINING 1. Must have a current and valid license as a Licensed Vocational Nurse issued by the State of California. 2. BCLS upon hire and maintain current. 3. ACLS and PALS preferred. 4. Minimum 1 year experience in an Acute Care Medical-Surgical setting or Long Term Care 5. Completion of or willingness/ ability to complete CDC infection prevention course. 6. MDs and Case Management experience preferred.
