We are seeking dedicated and experienced Clinical Nutritionists (RDs, LDNs, CCNs, and/or CNSs) with a focus on Functional Medicine to join our healthcare team. The ideal candidate will have a passion for holistic health, a deep understanding of nutrition, and the ability to create personalized, evidence-based treatment plans. This role involves working closely with patients to address the underlying causes of disease through diet, lifestyle, and nutritional interventions under the supervision of our Functional Medicine doctors.
While Aligned Modern Health is a Chicago-based company, this role is currently remote. We prefer Illinois-based candidates, but qualified remote candidates who hold an active IL license will also be considered. Compensation for this role ranges from $65,000 to $75,000 per year, based on experience.
Job Responsibilities:
• Perform patient assessments and consultations.
• Conduct comprehensive nutritional assessments that consider the patient's history, lifestyle, and biochemistry.
• Collaborate with patients to understand their health goals, challenges, and dietary preferences.
• Utilize Functional Medicine principles to identify root causes of health issues and design individualized nutrition plans.
Treatment Planning:
• Develop and implement personalized nutrition plans that incorporate whole foods, supplements, and lifestyle changes to support optimal health.
• Educate patients on the relationship between nutrition, lifestyle, and overall health, emphasizing the role of diet in preventing and managing chronic conditions.
• Monitor and adjust treatment plans based on patient progress and evolving needs.
• Actively manage patients across their care plan to achieve the desired health outcomes.
Collaboration and Communication:
• Collaborate with our Functional Medicine doctors and the broader cross-functional provider team to provide integrated care.
• Communicate complex nutritional information in a clear and supportive manner to patients and their families.
• Stay informed on the latest research and trends in Functional Medicine and nutrition.
Documentation and Compliance:
• Maintain accurate and up-to-date patient records, documenting assessments, treatment plans, and progress.
• Ensure all practices comply with current regulations and ethical standards.
• Participate in ongoing professional development and continuing education to maintain licensure and expand knowledge.
Requirements:
• Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited institution.
• Certification: Registered Dietitian Nutritionist (RDN), Licensed Dietitian Nutritionist (LDN), Certified Clinical Nutritionist (CCN), or Certified Nutrition Specialist (CNS).
• Licensing Requirement: Active Illinois license in one of the above certifications is required to start employment.
• Minimum of 3 years of experience in clinical nutrition, with a focus or interest in Functional Medicine.
• Must have flexibility to work a non-traditional schedule, including some early morning, evening and weekend availability.
Skills:
• Strong understanding of Functional Medicine principles and the ability to apply them in a clinical setting.
• Excellent communication and interpersonal skills.
• Proficient in developing and implementing dietary plans tailored to individual needs.
• Ability to work independently and as part of a multidisciplinary team.
Preferred Qualifications:
• Certification or training in Functional Medicine (e.g., Institute for Functional Medicine).
• Experience working with patients with chronic conditions such as autoimmune disorders, gastrointestinal issues, hormone imbalance, and metabolic syndromes.
• Knowledge of dietary supplements and their appropriate use in clinical practice.
