Requirements:
New York State LMSW or LCSW
Screen certified preferred, in-patient hospital, med-surge.
Assessments, discharge planning, resource finding, supportive counseling
Required 1 year minimum experience of hospital related social work

Experience with long term case planning and developing care plans
