A health services organization in California is looking to add a new Licensed Practical Nurse (LPN) for a promising opportunity with their growing team. In this role, the Licensed Practical Nurse (LPN) will be responsible for providing basic nursing care, including wound care, recording vital signs, collecting specimens ordered, and preparing clients for physical exam rounds.
About the Opportunity:
- Start Date: ASAP
- Assignment Length: Ongoing
- Shift: Days
- Hours: 8am to 4:30pm
- Setting: Adult Day Program
- Patient Volume: 3 to 6 per day
Responsibilities:
The Licensed Practical Nurse (LPN) will:
- Administer and manage controlled drugs per state regulations and Momentum policies and procedures
- Conduct walking rounds to determine persons supported status every two (2) hours and as needed, includes beginning and end of shift
- Prepare medications for one scheduled administration time
- Communicate persons supported change of condition directly to the Physician when Registered Nurse is unavailable
- Document in timely fashion treatments or nursing interventions entering into persons supported chart and/or therapy, as appropriate
- Assist in initial evaluation of persons supported upon admission
- Escort persons supported to dental and medical appointments as need arises.
- Complete all nursing documentation on a timely basis and by the end of shift.
- Perform other duties, as needed
Qualifications:
- 6 months of Nursing experience.
- Graduate of an accredited School of Nursing
- CA Licensed Practical Nurse (LPN)
- G-Tube experience
Desired Skills:
- 6+ months of experience working with Adults that have Mental / Physical disabilities
