Job Overview - Licensed Practical Nurse:
Our esteemed healthcare client in New York City is currently in search of a compassionate and highly skilled Licensed Practical Nurse. This role involves overseeing medical care management for adolescents aged 14 to 21 in residential and family foster care settings.
Location: New York, NY
Compensation: $45/hour
Schedule: 9:00 AM to 5:00 PM, Monday to Friday

Responsibilities for Licensed Practical Nurse:
- Identify Health Needs: Conduct direct interaction and home visits to understand the unique health requirements of adolescents and their caretakers.
- Establish Medical Home: Facilitate the creation of a medical home environment to address the comprehensive healthcare needs of adolescents.
- Serve as Liaison: Act as a bridge between multiple healthcare providers to ensure cohesive and comprehensive healthcare delivery for adolescents in care.
- Provide Support: Offer emotional, psychological, and spiritual support to adolescents and their families throughout their healthcare journey.
- Document Care Services: Maintain meticulous documentation of all care services provided, ensuring compliance with regulatory standards.
- Administer Medication: Safely administer medication and closely monitor the response to prescribed treatments.
- Integrate Sanctuary Model: Incorporate the Sanctuary Model SELF framework into daily practice to ensure quality care and an effective work environment.
- Other Duties: Fulfill any additional responsibilities as assigned to support the holistic care of adolescents in residential and family foster care settings.
- Experience: Graduate of an accredited practical nursing program.
- Certification: Current American Heart or American Red Cross CPR Certificate
- Skills: Excellent verbal and written communication skills and ability to work collaboratively with clients, family, and staff.
