Licensed Practical Nurse (LPN) - Primary Care Annual Wellness Program - University of Wisconsin Medical Foundation

Full-time, 90% FTE, day shift. Monday - Friday between the hours of 8:00 AM - 5:00 PM, no weekends required. Hours may vary based on the operational needs of the department.

Pay :

UW Health offers a competitive compensation package. Work experience that is relevant to the position will be taken into consideration when determining the starting base pay. External hires may be eligible for up to a $2,700 sign-on bonus (pro-rated based on FTE).

We are seeking a Licensed Practical Nurse(LPN) to:

• Join the UW Health Annual Wellness Team.
• Conduct annual wellness visits via telehealth including risk assessments and capturing HCC (Hieratical Condition Category) data.
• Promote healthy lifestyles, wellness and education.

Qualifications

• High School Diploma or equivalent Required
• Certificate from accredited Practical Nurse Program Required

Work Experience

• 1 year of experience as an LPN Preferred

Licenses & Certifications

• Wisconsin Practical Nurse Licensure Required
• CPR/BLS certification Required
