WCS Healthcare is currently hiring an LPN for a large Social Services agency in Brooklyn working with the underserved population/Homeless Population. This is a full time 3 Days 12 Hour Shift role!

Job Function:

• Day Shift
• Wednesday, Thursday, Saturday 8am - 10pm with 1.5 hour lunch break  
• Make referrals and suggestions for medical treatment, psychiatric rehabilitation, and coordinating information with treating physicians, pharmacies and other treatment collaterals.
• Manage client health needs by conducting health and vitals checks and coordinating with nursing leadership and other team members as needed to optimize client outcomes.
• Share findings and client needs in treatment team meetings and case conferences and through chart documentation and the maintenance of the onsite medication administration record and controlled substance log, including med counts and follow up to ensure adequate medication supply.
• Work directly with clients to provide health education and to learn and advocate for client health preferences and the development of client health goals through partnership with other team members at the site.
• Provide emergency first aid/CPR assistance when needed. Related duties as assigned.

Job Requirements:

• LPN Certification
• BLS through American Heart Association
• Must be willing to work with the Homeless Population in NYC
