WCS Healthcare has an immediate opening for an RN Case Manager with a large Social Services Agency for an Office of Mental Health Respite Program in Manhattan. The Medical Respite program offers a short-term housing solution where patients can complete their recovery after being discharged from the hospital. During their stay, patients receive medical care and other supportive services aimed at reducing preventable re-hospitalizations and Emergency Room visits.
Job Function:
- Performing psychiatric assessments, evaluating physical health needs, referring patients to community physicians as needed, managing and administering medication in collaboration with the psychiatrist, and delivering various treatment, rehabilitation, and supportive services.
- Create, update, and revise written care plans and general health care records.
- Conduct outreach to engage new clients in the continuum of care.
- Work with other health care providers and make appropriate referrals.
- Act as a client advocate, particularly in accessing the health care system.
Job Requirements:
- NYS RN License
- Experience working with the Mental Health, Substance Abuse or Homeless population preferred
- Hospital Discharge experience required
