***PLEASE NOTE: There will be no OT rate for this assignment request. Any time outside the regular hours will be billed at the regular rate***

MUST have active DEA & License. Only list the license number in the field, not an IMLC letter number.
(provider must be 90+ miles from facility)
-*NRP, PALS,ACLS, ATLS is required if not ABEM.
If an Emergency Department provider is ABIM or ABFM, 2500 hours of ED experience needed & they would also need to have ATLS.
EM providers need to have taken ATLS once if they are ABEM
PROVIDER MUST BE ER,FM,IM RESIDENCY
If your bid is accepted, you must submit the credentialing documents by the deadline given by the onboarding coordinator. All credentialing documents are in the "other documents" tab. ***You can start this process before your bid
is accepted***
Provider must be contracted with the agency prior to placing a bid.
An active license IS mandatory for every bid. Only list the license number in the field, not an IMLC letter number.
