About the Facility:
This well-established Non-Profit Skilled Nursing and Rehab Center provides exceptional care to over 100 residents. The facility features a busy short-term rehab unit, averaging around 20 Med A and Managed Care residents at any given time. With a focus on both long-term care and rehabilitation, we are dedicated to delivering high-quality, resident-centered services in a supportive and collaborative environment.
Position Overview:
We are seeking a skilled and detail-oriented MDS Coordinator to manage and oversee the Minimum Data Set (MDS) process for a very well staffed, non-profit community. The MDS Coordinator plays a crucial role in assessing residents' needs, ensuring accurate documentation, and supporting the interdisciplinary team in providing the highest level of care.
Key Responsibilities:
- Coordinate and manage the completion of the MDS assessments in accordance with federal and state regulations.
- Collaborate with nursing, therapy, and other interdisciplinary team members to ensure accurate and timely documentation.
- Monitor resident care plans and ensure they reflect the individual needs of each resident based on MDS assessments.
- Work closely with billing and reimbursement teams to ensure accurate coding and optimize reimbursement.
- Ensure compliance with all regulatory standards related to MDS assessments and resident care.
- Provide education and training to staff on MDS-related processes and documentation requirements.
- Participate in care plan meetings and ensure that residents' clinical and psychosocial needs are met.
Qualifications:
- Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of Connecticut.
- Previous experience as an MDS Coordinator in a skilled nursing or long-term care facility.
- Strong knowledge of Medicare, Medicaid, and Managed Care reimbursement processes.
- Familiarity with MDS 3.0, care planning, and regulatory compliance.
- Excellent attention to detail and organizational skills.
- Ability to work collaboratively with an interdisciplinary team.
