About the Job
Responsibilities:
- Utilizing clinical social work techniques and theory, consults on, assesses, and intervenes with high-risk patients and their families regarding the impact/amelioration of emotional and social problems.
- Duties also include, but are not limited to, conducting psychosocial assessments, making a psychosocial diagnosis, planning and implementing effective treatments, evaluation, education, supervision, coaching and mentoring of the patient (client) and family members.
- The position consults with, collaborates and communicates with a wide range of social, governmental and legal agencies, courts, schools, clinics, other hospitals, physicians and other sources in the overall care of the patient.
- This is a medical social work department we are paired alongside nurse case managers who support with discharge planning.
- We assess social determinants of health by completing biopsychosocial assessments with the goal of increasing access to quality health care and reducing barriers to discharge.
- We provide patients and families with emotional and resource support by providing interventions that are strength-based, focused on the whole-person, trauma informed and span across the life cycle of our patients
- We collaborate with the multi-disciplinary teams on how the illness or injury may impact the patient’s, family, employment and living situation.
- We promote health and reduce health inequities by advocating for, educating and empowering patients and families.
