Job Summary:
The Providence Center Child and Family Services Healthy Transitions Manager will provide overall clinical and administrative support to the Healthy Transition team. Coordinate, train, and provide ongoing clinical supervision for existing and new program services. Oversee intake process, case assignment and performance improvement monitors.
The Health Transitions Manager directly supervises 8 to 12 employees on the team. Carry out supervisory responsibilities in accordance with TPC policies and applicable laws. Responsibilities include planning, assigning, and directing work; addressing complaints and resolving problems.
Duties and Responsibilities:
Supervise the daily operations:
Interview, hire, and supervise staff,
Assign cases and monitor staff caseloads and productivity,
Oversee the intake process,
Monitor completion of progress notes,
Ensure the quality and timely completion of Health Information Services (HIS) and other documentation.
Perform audits of client records, including reverse audits, in accordance with Center policy.
Address areas of concern with staff and develop performance improvement plans when needed.
Process staff time sheets,
Screen referrals,
Serve as contact regarding questions from BHDDH and attend all agency meetings and BHDDH meetings
Coordinate services with community partners as needed,
Collaborate with other providers, agencies, and individuals,
Chair Team meetings,
Maintain accurate program census,
Monitor compliance of BHDDH regulations,
Submit appropriate BHDDH spreadsheets as needed.
Supervise the operations:
Apply knowledge of Psychiatric Diagnosis (DSM IV).
Apply knowledge of psychopathology and primary methodologies of psychotherapy.
Use psychiatric consultation and intervention when clinically appropriate.
Use knowledge to decide appropriate levels of care in order to refer to various points in the continuum of care.
Demonstrate awareness and skill in addressing areas related to diversity.
Provide supervision including clinical supervision, performance monitoring, and evaluations.
Assist in planning meetings, case conferences and training of staff.
Act as a resource on client treatment issues for Program Director including staff development needs.
Ensure required reports are completed in a timely fashion.
Ensure TPC policies and the program's standards of care are maintained.
Monitor contract and licensing obligations ensuring compliance with regulations.
Ensure all CARF standards are being maintained.
Monitor operational cost to insure fiscal integrity/develop and adhere to annual budgets.
Promote communication and coordination of services.
Monitor compliance with finance and petty cash policies and procedures.
Recommend program terminations and coordinate in accordance with Agency and referral source policies.
Maintain knowledge of the systems, services and resources available to consumers and access them as necessary.
Provide on-call coverage as needed in case of illness or vacation of the assigned on-call staff person.
Assist in the development and implementation of program monitoring, evaluation and research.
Attend mandatory in-service trainings and other trainings as required for licensure.
Maintain active involvement in professional and lay mental health organizations within the Community.
Maintain cooperative relationships with consumers, other employees and the public.
Communicate with various local agencies and other departments or teams at The Center.
Work in cooperation with co-workers and supervisory staff.
Deal with team members regarding problems in a constructive manner.
Accept and use direction and supervision.
Perform in a calm, professional manner in all job related situations.
Perform other duties as assigned by supervisor.
Oversee referrals, case assignment and census.
Complete record keeping in accordance with Health Information Services within required time frames and other TPC requirements.
Develop and implement a plan of care with ongoing consumer input.
Transport consumers in own vehicle or TPC van to planned activities and appointments.
Maintain cooperative relationships with other employees, consumers and the public.
Attend trainings and conferences.
Serve on appropriate Center committees.
Requirements:
Masters degree in Human Services field required.
Two years relevant experience in direct care with the psychiatrically disabled population and/or in a similar social service agency, as well as 2 years prior supervisory experience.
Bilingual Spanish preferred.
Must have valid driver's license and insured auto.
Must obtain QMHP while in position.
Ability to read and interpret documents such as policy and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before team members and other employees and have the capacity to work with other community agencies.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers.
Ability to define problems, collect data, establish facts, and draw valid conclusions.
Must be able to be flexible.
