The Medical Records Clerk 2 ensures all forms are properly identified, completed, and signed. Enters all necessary information into the system. Communicates with physicians and staff to clarify diagnoses or get additional information. May also assign a code to each diagnosis and procedure. Decisions are typically focus on interpretation of area/department policy and methods for completing assignments. Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction. Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.

The Medical Records Clerk assembles and maintains patients' health information in medical records and charts. The Medical Records Clerk performs varied activities and moderately complex administrative/operational/customer support assignments and computations. Typically works on semi-routine assignments. Collaborates with medical record vendors, center operations and process improvement teams, and communicates with physicians and clinic staff to clarify diagnoses or request and obtain additional health information. The Medical Record Clerk may also assign a code to each diagnosis and procedure. Decisions are limited to defined parameters around work expectations, quality standards, priorities, and timing. The Medical Records Clerk works under supervision of the Clinic Administrator.
Essential Functions:
+ Ensure all patient forms are completed and signed, and all patient health information is properly identified and entered in the electronic medical records system
+ Initiate EMR actions for the coordination and processing of medical records requests according to Federal/State/HIPAA guidelines, including but not limited to:
+ New patient record request
+ Hospital notes request (via form or hospital portal)
+ Patient self-records request
+ Subpoena request
+ Obtain pending specialist provider consult notes and diagnostic imaging reports
+ Obtain advance directives
+ Reconcile incomplete faxes when vendor(s) is unable to identify patients and maintains monthly logs for requests of patients not found within EMR
+ Update electronic medical records, including lab reports, correspondence, on-call dictation, etc., in proper order, following office guidelines
+ Create electronic medical record (EMR) alerts for privacy
+ Forward documents to covering physicians when PCP is out of office
+ Answer telephone calls regarding Medical Record questions in a friendly and knowledgeable manner
+ Local travel is required; Mileage is reimbursed.
Required Qualifications:
+ This position will cover multiple clinics in the Tennessee market including but not limited to: Murfreesboro, Clarksville, and Lebanon
+ At least 1year of experience working in Medical Records
+ Excellent Customer Service
+ Must interact with patients in a respectful and amicable manner.
+ Employees are expected to treat patients with compassion and protect their rights.
+ Computer skills, scanning, experience requesting medical records from the hospital or specialist office
+ Must be well organized, detail oriented, and able to multi-task

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
$34,500 - $47,400 per year
This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
