TEAMWORK 

Medical Records Clerk Job Summary:

• Prepare quarterly resident discharge/death report and physician caseload report for QA and Administration.
• Maintain resident incident report files.
• Maintain record of MDS review dates for each resident and schedule weekly IDCP lists.
• Prepare forms for meetings and e-mail information to MDS team.
• Maintain Discharge Diagnosis code index for all discharge charts.
• Arrange for destruction of Resident’s records and other confidential material every six months.
• Maintain a record destruction log of all records destroyed.
• Maintain record room in a neat and organized manner.
• Order supplies for the department.
• Log requests for release of medical record information and follow up as needed.
• Maintain a current and discharge resident Rolodex.
• Type reports, file, Xerox and fax as needed.
• Assist with unit secretarial coverage if needed.
• Complete new admission sign in packets timely
• All other duties as assigned
