Schedule
8-hour day shifts, Friday-Sunday (24 hours/week)
Enjoy a shift differential of +$3.10/hour.
Position Summary
As a Medical Social Worker, you'll play a vital role in our interdisciplinary care team. You'll provide comprehensive psychosocial support to patients and their families at the McClure Miller Respite House and/or in the community hospice setting . Your expertise will help address emotional, social, and practical challenges, ensuring a seamless transition through their care journey.
Key Responsibilities
Conduct comprehensive psychosocial assessments to identify patient needs and barriers to care.
Develop and implement individualized care plans in collaboration with the interdisciplinary team.
Provide crisis intervention, counseling, and support to patients and families.
Coordinate with community resources to ensure optimal care and support.
Maintain accurate and detailed documentation.
Facilitate and participate in care conferences and family meetings.
Qualifications:
Master's degree in Social Work required.
Vermont LMSW or LICSW licensure preferred.
Minimum two years of relevant experience in healthcare.
Strong communication and interpersonal skills.
Ability to work independently and as part of a team.
Passion for providing compassionate care.
