Position Overview:
Sage Health is seeking a dedicated Cardiologist to join our team-based, value-driven care model, focusing on delivering high-quality, patient-centered care. Key Responsibilities:
- Deliver high-quality, evidence-based cardiology care to patients in line with our standards of care.
- Coordinate and manage patient services, including specialty referrals to cardiology subspecialists, vascular surgeons, and other relevant specialists.
- Serve as a clinical leader within the care team, providing medical expertise and fostering a cooperative environment.
- Collaborate with primary care physicians and nurse practitioners, maintaining professionalism and approachability.
- Document patient encounters daily in collaboration with quality coders to ensure comprehensive and accurate records.
- Adhere to protocols and policies set forth by Sage Healths Clinical Leadership.
- Participate in quality review meetings and collaborate with medical directors to ensure high standards of patient care.
- Provide direct patient care, including clinical consultations and administrative tasks directly related to patient care.
- Support our longitudinal care approach for conditions such as heart failure, focusing on disease management and preventive care.
- Lead initiatives to reduce readmissions related to heart failure and coronary artery disease.
- Develop and implement preventive measures for stroke and coordinate care for advanced chronic kidney disease (CKD) patients.
- Interpret and document diagnostic tests, including echocardiograms and stress tests, to assess cardiovascular function.
- Coordinate care with primary care providers and subspecialists, including interventional and electrophysiology cardiologists and thoracic surgeons.
Qualifications:
- MD or DO degree from an accredited institution.
- Current, unrestricted medical license in the state of practice, with a willingness to obtain licenses in other states within the region if required.
- Board Certification in Cardiology.
- Active and unrestricted DEA license.
- Medicare and Medicaid provider numbers.
- Strong interpersonal skills with a commitment to team-based care.
- Adaptable to practicing in a value-based care setting.
- Excellent patient and customer service skills.
- Basic proficiency in computer applications, including email and electronic medical records (EMR).
- Must be patient-facing and comply with TB screening protocols.
Preferred Qualifications:
- Knowledge of HEDIS/STARS measures.
- Familiarity with CAHPS and HOS.
- Experience in underserved or culturally diverse communities.
- Experience with Medicare Advantage Global Risk Models.
- Proficiency in electronic health record systems.
Additional Information:
- No on-call requirements.
- No weekend responsibilities.
- No pediatric care required.
- Reports to: Network Manager.
