Compensation: $52/hour
Location: Rochester, NY
Schedule: Monday to Friday, 2 days of home assessments, 3 days of telehealth (Flexible hours)

Responsibilities as the Nurse Assessor, Home Health (RN):
- Health Assessments: Conduct in-home evaluations of patients, assessing health status, care needs, and preferences to guide long-term care service plans.
- Documentation: Review patient medical documentation, record evaluation data into electronic systems, and identify any concerns during the assessment process.
- Care Coordination: Facilitate communication between patients, care teams, and managed care health plans to ensure seamless care coordination and service delivery.
- Health Planning: Discuss healthcare options, supports, and service vendor options with patients and help them understand available waiver options.
- Data Collection: Gather data to support quality reviews and track care outcomes, documenting case notes in the CRM.
- Education: Bachelors degree in Nursing (BSN) or an accredited RN program.
- Experience: 2+ years in Home Care with CHHA, LHCSA, or MLTC.
- Assessments: Skilled in conducting UAS or OASIS assessments and care planning for homebound or elderly patients.
- Technical Skills: UAS-NY experience (roles 40 or 45) within the last year preferred.
- Population: Experience in managing chronic diseases and geriatric care in home settings preferred.
- Home Care Background: Prior experience as an LPN or paraprofessional with RN home care experience preferred.
