Nurse Practitioner Acute Care
Shift: Mondays through Fridays, no weekends, evenings, or holidays
Status: 1.0 FTE
Unit: Hem/Onc Clinical Services
Experience: Hem/Onc/BMT experience and CPHON certification preferred or sought within 3 years

The APP in this role will work with the anticoagulation team to help manage patients who are receiving anticoagulation therapy across the hospital in both the inpatient and outpatient settings. In the role the APP will help to manage inpatient anticoagulation consults as well as manage anticoagulation medications in the outpatient setting. The ideal candidate for this role has experience in pediatric Hem/Onc/BMT and/or has experience in managing anticoagulation therapy. CPHON certification is preferred but not required for hire. If not certified at time of hire would seek APP to be certified after 3 years in the role. The candidates that will be most fitting for this role have strong communication and collaboration skills and can work well with a multi-disciplinary team.
This position involves working Monday through Friday but does not include weekends, evening or holidays.
Provides healthcare to individuals or groups that requires knowledge and skill obtained from advanced formal education, certification and clinical experience. Practices within professional scope and promotes patient advocacy in accordance with the profession?s Code of Ethics and maintains a current Ohio Advanced License.

Responsibilities:- Assesses patients and/or populations; develops, implements and evaluates plans of care; clearly documents and communicates information both in written and verbal forms.
- Performs diagnostic and therapeutic procedures according to established guidelines and standards as necessary for the care of the patient

EDUCATION: Master of Science degree or equivalent provision
LICENSURE: Licensed as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in the state of Ohio
CERTIFICATIONS: Maintains national certification as APRN in the appropriate population foci for the position, or PA
MINIMUM PHYSICAL REQUIREMENTS
Note frequency as Constantly (67-100%), Frequently (34-66%) and Occasionally (0-33%) for each requirement. Also note any lifting requirements with pound limit.
