The Nurse Practitioner delivers primary health care and patient care management in  residential and assisted living facilities. The Nurse Practitioner manages reasonable daily case load via face-to-face visits. You will have a company vehicle + assistant.

Qualifications

• Experience in Geriatrics or complex patients preferred.
• Active/unrestricted medical license and DEA
• Board Certified
• EMR experience
